Home Care Packages (HCP) Level 1 - 4

Aged Care Reforms were introduced as from 27th February 2017

What does that mean for you?

It has created new opportunities for older people and their families. It is a step forward in Consumer Directed Care. You now have more choice about the care you receive at home and who delivers it. Home Care Package recipients are now able to select the provider and services that best suit their individual needs. If you are not happy with your current provider you are now able to change to a different provider easily.

 

How can you receive a Package?

To access a Home Care Package you need to contact My Aged Care -  www.myagedcare.gov.au or phone 1800 200 422.  

An assessment will be conducted to determine eligibility for a package. There are four levels of packages/ funding available dependant on the recipient’s needs.

If you are allocated a package you may advise My Aged Care that you would like NDNS to be your provider. In addition if you are currently receiving services under a package and would like to change providers we can assist you through this process.  

 

How can NDNS assist you?

It is important that you are happy with your chosen provider and the services they are delivering to you.

Our Care Coordinator will discuss all your options with you, working together with yourself, your family and a care team member to develop an individualised care plan.

These services may include personal care, respite care, social support, companionship, and domestic assistance.  We discuss what is important to you and how we can assist you to maintain your lifestyle and achieve your goals.

Fees

NDNS charges a 20% Care Management Fee (Applicable to all Levels 1, 2, 3 and 4 only) and 5% Plan Management Fee (Applicable to Levels 1, 2, 3 and 4 only)

Daily Care Fee is optional and fee free.
