Support at Home

Support at Home is the Australian Government program that replaced Home Care Packages from 1 November 2025, helping older people stay independent and live well in their own homes.

What does that mean for you?

Support at Home (formerly Home Care Packages) puts you in control of the care you receive at home and who delivers it. You can choose the provider and services that best suit your individual needs, and if you are not happy with your current provider, you can change to a different one.

How do you get started?

To access Support at Home you need to contact My Aged Care: www.myagedcare.gov.au or phone 1800 200 422.

An assessment will be conducted to determine your eligibility and the level of funding to suit your needs.

Once you are approved, you can advise My Aged Care that you would like NDNS to be your provider. If you are already receiving support and would like to change providers, we can assist you through this process.

How can NDNS assist you?

It is important that you are happy with your chosen provider and the services they are delivering to you.

Our Care Coordinator will discuss all your options with you, working together with you, your family and a care team member to develop an individualised care plan.

These services may include personal care, respite care, social support, companionship, and domestic assistance. We discuss what is important to you and how we can assist you to maintain your lifestyle and achieve your goals.
