Coordination
Care coordination is the thread that holds everything together: managing your plan, your services, and the people who help you.

When several services and people are involved, one point of contact who knows your story makes all the difference. NDNS provides care coordination across Newcastle, the Hunter and the Mid North Coast.
A single point of contact
Care coordination is provided by a Registered Nurse or Cert IV who works in consultation with the client, carer and family. The coordinator uses a collaborative approach: assessing your needs, holistic care planning, coordinating multiple services, and regularly reviewing your wellbeing. This helps clients with complex care needs live as independently as possible in their own home.
When coordination helps most
- You are juggling several services or providers
- Your needs are changing and the plan needs to keep up
- Family are involved and want to stay informed
- You want one person who knows the whole picture
Getting started
Coordination is available through Support at Home, the NDIS and privately, and works hand in hand with nursing care and personal care. Call our team to talk it through.
